CT Colonography
What is CT Colonography?
CT colonography is a screening technique used in the diagnosis of colorectal disease. CT Colonography is a test that uses CT scan and computer software to look inside the large intestine. It is different than conventional ways of colon screening in that it does not require sedation or filling the colon with barium. The entire colon is assessed for polyps by acquiring hundreds of thin CT images of the abdomen and pelvis and processing the data on a 3D workstation.
What are the risk factors and symptoms of colorectal disease?
Risks factors for colorectal cancer include a family history of cancer in the colon or rectum, personal history of cancer in the colon, rectum, ovaries, endometrium or breasts, history of polyps in the colon, and history of ulcerative colitis or Crohn's disease. Symptoms of colorectal cancer may include a change in bowel habits, blood in the stool, general abdominal discomfort and unexplained weight loss.
Why is colorectal cancer screening important?
Colorectal cancer is second only to lung cancer as a cause for cancer related deaths in the United States. There are approximately 150,000 new cases of colorectal cancer every year in the United States resulting in nearly 55,000 deaths. The goal of all colorectal screening is to locate polyps, small growths in the colon which are the precursors of cancer. For polyps of 1cm in size, there is an approximate 1% chance of harboring cancer and for polyps smaller than this it is likely closer to 0.5%. Unfortunately, while effective screening is available, the majority of those individuals eligible for screening do not have it performed. In most cases of failed or incomplete optical colonoscopy, CT colonography is the imaging procedure of choice. In low to average risk patients who cannot or will not undergo traditional optical colonoscopy, we believe CT colonography to be the best alternative.
How is a CT colonography performed?
The exam requires a 24 hour cleansing bowel prep in order to remove bowel residue that may interfere with polyp detection. Information regarding the type of prep will be provided when your exam is scheduled. You may pick up a prepared bowel prep kit in the radiology department of NorthEast Medical Center. When you arrive in the department you will be asked to complete a brief patient history sheet and then escorted to the CT scanning area where you will put on a gown. Once lying on the CT scan table, a small catheter will be inserted into the rectum in order to fill the colon with air. Many patients report a sensation of fullness or mild discomfort. Most patients consider CT colonography much less uncomfortable than a barium enema, and nearly 50% rate the level of discomfort the same or less than traditional colonoscopy. A small number of patients experience cramping. Once the colon is filled with air, scans will be obtained while you are on your back and then with you on your stomach. Since no sedation or pain medication is required for CT colonography, once the images are complete, you are free to leave the radiology department and resume routine activity.
